For your convenience, you may download our Adoption Application. Complete it and return it to us via U.S. Mail or by attaching it to an eMail to us. Our mailing and eMail addresses are on the application.
The application is in PDF format. You must have the Adobe Acrobat Reader in order to view the application document. If you do not have the Reader, please download it FREE from the Adobe web site. Install it and then return to this page. To view the application, left click the application link above. To download, right click the application link and save to your hard drive.
Submitting your application is a required pre-requisite to meeting or adopting the dog(s) you may be interested in. No dogs are adopted out until we receive, review and approve your application. Our organization is comprised of volunteers. Please understand that it may take a few days for us to contact you after we receive your application.
A home visit and interview with all family members residing in the home is also required before applicants will be permitted to meet the dog(s) they're interested in. If you have other pets, we will evaluate how your selected dog interacts with your existing pets before an adoption is approved.
If the dog you're adopting is not already spayed or neutered, it will be adopted under a "Spay/Neuter Contract." This means the dog must be spayed or neutered within 30 days of the date of adoption. You will be required to return proof of the spay/neuter. When the veterinary certificate is received by CWGSR, you will receive your spay/neuter deposit back.
